All fields on the left form, are required except the one labled '* Other' as long as the selected value in the 'Your Title' field is not 'Unspecified Other'. The field '* Other' becomes required upon selecting the last option 'Unspecified Other'.

The field labeled 'Interfaces' represents the number of lab equipments you would want to interface with Prolis.

Interfaces field is to hold the number of equipments to be interfaced with prolis. This field should have '0' for no equipment to interface.

Web Outreach is the website for your laboratory created by Prolis, for your clients to accession their specimen and access results from within their office.

Users field is the number of prople expected to use Prolis.

The field 'Av Daily Sp,' is the average number of specimens handled by your laboratory on daily bases when the lab is open for business.

Note (Equipments) field is required to be used to list the Equipment names with respective manufacturer's names if the Interfaces field has the value greater than zero (0).
